You may have noticed that private insurance can be expensive and does not always cover everything you need. The good news is that you have the option to enroll in the Italian public healthcare system (Servizio Sanitario Nazionale – SSN) as a self-paying member.

What benefits do you get as a self-payer of SSN?

  • Full access to the Italian healthcare system – you receive the same benefits as Italian citizens – doctor visits, hospital care, specialist consultations, and prescription medications.
  • Family doctor (“Medico di Base”) – after registering, you can choose your general practitioner, who will provide medical care and refer you to specialists if needed.
  • Lower healthcare costs – Instead of paying high private fees, you will only need to cover a small co-payment (“ticket sanitario”) for specialist visits and medical examinations.
  • Discounts on medications – public health insurance covers a significant portion of medication costs – some are even completely free or available for a minimal fee.

How to Register for the Italian Healthcare System as a Self-Paying Member?

1. Deregistration from Your Current Health Insurance

If you decide to be insured in Italy, you must deregister from your current health insurance provider. It is not possible to have basic health insurance in two EU countries simultaneously.

2. Registration at Azienda Sanitaria Locale (ASL)

Visit the nearest health office (ASL) and provide the following documents:

  • Identity card or passport
  • Codice Fiscale (tax identification number)
  • Proof of residence in Italy (residenza anagrafica)
  • Proof of income or a declaration of financial self-sufficiency
  • Proof of payment for the insurance fee

3. Payment of the insurance fee via F24 form

The fee is calculated based on income:

💰 Minimum €700 per year (for individuals with low income)
💰 Maximum €2,000 per year (for higher income)

4. Activation of Health Insurance

After registration, the ASL will issue a confirmation and a health card (Tessera Sanitaria), which you will use for doctor visits.

5. Annual Renewal of Insurance

The insurance is valid for the calendar year (January – December) and must be renewed annually.
